Accessibility Accommodation Relevant Information
The student must provide current (within 5 years), written documentation from a medical physician and/or licensed clinician that verifies their disability with recommended accommodations.
- Documentation provided to the Student Accessibility Services is reviewed on a case-by-case basis.
- Recommendations by a licensed professional must be reasonable and effective in assisting the student where the student’s major life activity/activities is/are impaired.
The Academic Accommodation must be completed by the student. If assistance is needed to complete the application, please email Accessibility Services.
A physician's or qualified evaluator's detailed letter on letterhead stating your diagnosis and the specific accommodation(s) needed in relation to that diagnosis must be provided.
- The detailed letter can be uploaded within the "Accommodation Request Application", emailed to accessibility@mc.edu, or dropped off at the Accessibility Services office located in the basement of Nelson Hall
- The clinician may email supporting documentation to accessibility@mc.edu

Documentation Release Request
- Student Accessibility Services will not share the student’s documentation to anyone except to the student and/or a designated individual at the student’s request via the Authorization to Release Information form.
- Instructions for the submission of the Authorization to Release Information form will be within the form.
- The student must provide adequate information regarding the reasoning for the request in order to best fulfill the student’s request.
